Please fill out this form NO LATER THAN June 30 to qualify for no cost increase for tuition.  Please note that payment does not need to be completed by June 30, just submission of the form.

Given the overriding Jewish value that puts a premium on the maintaining of health, including the taking of preventative measures, along with the clear public health based need to protect the community as a whole, we are requiring that all children enrolled at Temple Beth Israel be immunized in the manner determined by their pediatrician and other health professionals that is in concert with New York guidelines.  Because we care about the health of all members of our community, immunization is required in order to attend our school, except for medically-necessary exemptions.

Please provide the name and number of a local friend or family member who can be contacted in the case of emergency (if both parents are unreachable).

TUITION

Kindergarten - 1st Grade:  $750 (Will be increased to $850 if you enroll after June 30)

2nd - 7th Grade - $1,250 (Will be increased to $1,350 if you enroll after June 30)

TBI High - $750 (Will be increased to $850 if you enroll after June 30)

Please select the number of learners in each category from the drop down menus below.  When this form is submitted, the office will be notified to bill you for tuition.  Payment is not required by June 30, just the submission of this form.
